This is an easy to install and customize email form script. Set required fields, confirmation email text, and human readable field descriptions for the email that comes to you. Submissions can also be logged on the server. The script is free, though there is an optional anti-spam module, which asks the user to answer a simple math equation, to verify they are human (and can add two numbers correctly!) and not a spam bot. The equation changes each time the script is loaded. NEW - you can have the script pause a few seconds if the wrong answer is given, to slow down robot scripts.
